Want to help shape healthcare in your community? Please take a few minutes to complete the survey for the Navajo County Community Assessment. This is an extremely valuable tool to help improve healthcare and safety in Navajo County! All responses will be kept confidential.
We also have a paper version that can be completed and submitted to Navajo County Public Health Services District, Attn: Bill Ricci. Stop by any of the NCPHSD offices and they will be happy to assist you.
